Provide appropriate pain / symptom management. Evaluates the patient's response to treatments / medications
JourneyCare Hospice is seeking a Hospice LPN to join our growing Crisis Care team! This is a field-based position responsible for following the plan of care for our continuous care patients, providing treatments, and working collaboratively to help our patients and their families.
HOURS : Monday - Friday (3) 12-Hour Shifts 7PM-7AM and Every Other Weekend
TERRITORY : Mundelein, IL 60060
Salary : $26 - $38 hourly - Offer Based on Years of Experience
Qualifications :
- Graduate of an accredited practical nurse or a vocational nursing program
- Three years of nursing experience. Community health, hospice or medical / surgical experience is preferred
- Currently licensed as an LPN / LVN in the practicing state
- Must possess current CPR Certification
- Demonstrates good verbal and written communication, and organization skills
- Valid driver's license with an automobile that is insured in accordance with state requirements
- Understands the philosophy of hospice concept and the needs of the terminally ill
