We're growing and looking for an experienced Registered Nurse (RN) to join our Hospice Care team!

What You'll Do

As a Hospice RN , you will plan, organize, and direct hospice care while educating patients and families. You'll serve as a case manager and ensure quality, compassionate care in the home setting.

Key Responsibilities :

• Complete initial and ongoing patient and family assessments
• Develop and revise individualized hospice care plans
• Provide professional nursing care using the nursing process
• Administer medications and treatments as prescribed
• Educate and counsel patients and families on hospice care
• Coordinate care for your assigned caseload
