Weekend LPN - Memory Care | Chapters Living of Tulsa

Chapters Living of Tulsa is seeking an experienced Licensed Practical Nurse (LPN) to support our small memory care community on Saturdays and Sundays . In this weekend lead nurse role, you'll provide guidance to staff, assist residents, and help ensure the highest standard of care and comfort. If you're a dependable nurse who enjoys a close-knit, positive environment-this is the perfect opportunity to make a meaningful impact each weekend.

Assessing and monitoring the health status of residents.
Administering medications and treatments as prescribed.
Assisting with activities of daily living such as bathing, dressing, and grooming
Documenting resident care plans and progress notes.
Communicating with healthcare professionals and family members.
Monitoring vital signs and reporting any changes to the healthcare team.
Ability to work independently and as part of a team.
Excellent communication and interpersonal skills.
Strong organizational and time management skills.
Ability to prioritize tasks and manage multiple responsibilities.

Requirements

Valid LPN license in the state of practice is required.

Minimum 1 year of experience in a healthcare setting, preferably in assisted living or long-term care is preferred.
