Progressive Life Center, Inc. (PLC) provides culturally-sensitivity human services to children, youth and families in the District of Columbia and surrounding Maryland, throughout Greater Philadelphia, and in most of Delaware. PLC’s array of community-based services includes foster care and adoption, independent living for youth aging out of foster care, family preservation, juvenile diversion, and parenting skills training. Program Summary
PLC is distinguished as the only private-care organization that provides Kinship Care services in Prince George’s County, Maryland. For families who are caring for a relative’s child or children, we help ensure stability and safety through case management, training, and the provision supportive services. Our Kinship Care Program is a virtual and home-visiting program specifically designed to provide these selfless caregivers with needed emotional support, guidance, counseling and financial assistance. We increase the likelihood that children will remain in the care of loving relatives – and prevent the necessity for placement in foster care.
Position Responsibilities
- Provide relevant crisis intervention and advocacy service and referrals.
- Maintain a caseload of 8-10 families.
- Conduct assessments of program participants upon admission to program.
- Provide 24-hour, 7-days a week coverage.
- Provide family / individual counseling.
- Administer funds to families, when appropriate.
- Provide case management and referral service; provide and link families to follow-up services.
- Provide in-home services in your own transportation, if necessary.
- Attend and participate in weekly supervision and case conferences.
- Attend and participate in interdisciplinary team meetings.
- Develop service plans, progress reports, termination summaries and follow-up documentation.
- Serve at least a one-year term on the Performance Quality Improvement (PQI) committee.
- Enter and maintain accurate data into database (e.g., FamCare) system.
- Participate in and complete the NTU retreat after 6 months of hire.
- Enter and maintain accurate data into database.
- Co-facilitate monthly support group.
- Other duties assigned.
Qualifications :
Bachelor’s degree in social sciences, education or related discipline from an accredited college or university is required.Two (2) years related field experience.Exceptional verbal and written communication skills.Strong basic computer skills and experience utilizing web-based case management programs.Knowledge of family systems and community resources.Experience utilizing FamCare preferred.Bilingual in English / Spanish is welcomed.Valid driver’s license and owner of reliable vehicle to travel within Prince George’s County and surrounding areas.Certification in NTU™ Service Delivery Model within one (1) year of hire.Physical Demands :

Must be able to remain in a stationary position 50% of the time.The person in this position needs to occasionally move about inside the office to access file cabinets, office machinery, etc.Constantly operates a computer and other office productivity machinery (i.e., a calculator, copy machine, mobile device and computer printer); Manual dexterity needed for using a mobile device, calculator and computer keyboard.Must be able to position self to stoop, kneel, crouch, or crawl.Must have the ability to navigate stairwells.The person in this position frequently communicates with employees / customers / clients ; and must be able to exchange accurate information by perceiving the nature of sound, near and far vision, depth perception, providing oral and written information.Frequently move files weighing up to 20 pounds throughout the office space.Must be able to participate in fire drills or other drills during an emergency.Must be available on call and often respond to situations during off hours which can disrupt normal sleep patterns and cause fatigue.Must be able to face a significant amount of emotional stress which can cause related physical stress on muscles as well as other physical symptoms such as headaches and fatigue.In cases of child relocation, the incumbent may be required to assist the child by carrying belongings and must often lift, carry and / or comfort small children.Work Environment :
Normal office environment or residential facility, but also may travel locally to visit clients within their homes, meet with service providers, and / or attend meetings : schools, hospitals, mental health clinics, private practices, prisons, corporations, and numerous public and private agencies.
