Care Manager(CNA / HHA) 2p-10p PT / PRN Every Other Weekend

We are seeking a dedicated and compassionate Care Manager (CNA / HHA) to join our team. This position offers flexibility with part-time and PRN (as needed) shifts from 2 PM to 10 PM, providing you with an excellent opportunity to make a meaningful impact on the lives of our clients while balancing your personal schedule.

• Responsibilities :
• Provide high-quality personal care and companionship for clients in a safe and respectful manner.
• Assist clients with daily activities, including bathing, dressing, grooming, and meal preparation.
• Monitor and record patients’ medical history, vital signs, and overall health condition accurately.
• Collaborate with healthcare professionals to plan and evaluate personalized care plans.
• Maintain a clean and orderly environment to ensure the safety and comfort of clients.
• Assist with mobility and physical exercises as prescribed by healthcare practitioners.
• Respond to emergencies promptly and effectively, ensuring the welfare of the client.
• Offer emotional support to clients and their families by maintaining open and positive communication.
• Uphold all company policies and procedures in accordance with healthcare regulations.
• Qualifications :
• Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification is required.
• Proven experience in a caregiving position is highly preferred.
• Ability to handle multiple tasks efficiently while maintaining exceptional attention to detail.
• Excellent interpersonal and communication skills.
• Compassionate, patient, and committed to providing the best possible care for clients.
• Ability to work independently while also collaborating effectively with a healthcare team.
• Flexibility to work the hours as specified, including weekends and holidays as needed.
