Purpose : The Field Nurse Case Manager facilitates and coordinates the medical and rehabilitation care of an injured or disabled individual in a timely and cost-effective manner; promoting optimal outcomes as well as physical and mental wellbeing, advocating for recovery from injury or illness, and helping them to move towards their optimal level of function.

Core Responsibilities :
- Assessing the individuals’ medical and functional issues related to an injury or disability and establishing, implementing, and coordinating the treatment / rehabilitation plan.
- Maintaining phone and / or digital contact with all parties involved in a timely manner.
- Coordinating and attending medical, therapy, in-home, and provider appointments as needed.
- Work with family members and pertinent parties regarding activity modifications to assist with recovery and developing independence, based on medical and functional limitations.
- Explain and educate the individual, care giver and family members with information provided by physicians and therapists, along with answering any questions the injured / disabled individual or their family / caregiver may have to facilitate recovery and maintain or improve functional status.
- Serve as an intermediary to interpret and educate the injured / disabled individual, care giver and or family members related to his / her injury and the treatment plan established by the case manager, physicians, and therapists, and other providers.
- Communicate with the supervisor and report on case status as needed.
- Acquiring and maintaining knowledge of developments in the medical case management field.
- Keep personal licenses, credentials, and accreditation current and in good standing in accordance with professional standards.
- Maintain documentation on all activities, complete letters, reports, and documentation on a timely basis.
- Document all time for case management duties as requested.
Requirements :
Licenses in Nursing, OT, LMSW, or industry or work experience deemed transferrable.CCM Certification (Certified Case Manager) preferred.Prior Case Management experience is strongly preferred.Strong medical / rehabilitation knowledge required.Willing to attend all provider, client, and rehabilitation appointments as needed.Excellent communication and writing skills.Strong computer skills for documentation and information exchange in remote work environment.Ability to multi-task.Strong understanding of medical terminology and rehabilitation.Demonstrated experience in hospital or rehabilitation settings.Familiarity with HIPAA regulations.Why VONA : We are driven by a shared purpose : to improve the lives of our clients and provide exceptional care in the Case Management Industry.
