Behavioral Health | Health Care Home Director
The Swope Behavioral Health -Health Care Home Program Director (BH-HCH), will direct, oversee, and facilitate all aspects of the BH HCH and Disease Management programs and services to include assurance to the quality, DMH regulatory, and accreditation requirements and standards for clinical practices. The Director will oversee and manage program associates and ensure competency and practices for client care are evidenced based and meet accreditation and regulatory requirements. The Director also ensures the documentation of care and services rendered, policy and procedures, and the accuracy and submission of internal and external reports and data required timely and accurately to reflect clinical program performance.
Primary Functions :
- Serves as the program / s leader for areas assigned to support scheduling, employee management, and facilitation of daily operations.
- Builds and facilitates new employee orientation and onboarding and ongoing competency program for current associates.
- Ensures accreditation readiness and leads team skills training and competencies as indicated.
- Ensures and oversees patient satisfaction process.
- Addresses and completes event reports as indicated.
- Builds and facilitates program annual performance goals and employee evaluations.
- Builds and deploys the quality assurance and performance improvement process for the programs assigned.
- Facilitates and leads monthly treatment team process and case review assignments for the specialized health care consultants in the program.
- Ensures completion of yearly client case reviews to meet DMH program requirements.
- Manages and facilitates clinical data entry into required platforms, and completes and submits reports monthly, quarterly, and as indicated by timeline required for compliance.
- Attends and participates in internal, external, and state level meetings, committees, and projects as required or assigned.
- May provide as needed, individual or group training and education to behavioral health associates.
- Performs other duties as assigned.

Registered Nurse with at least five (5) years experience management and case management experienceMaster's degree in Health Care Administration, Health Management or related field with at least five (5) years experience in a health care setting.Experience in quality improvement principlesDemonstrated experience in new project management development and successful implementationExcellent analytical and computer skillsAbout Swope Health Services :
