Position Summary

We’re seeking a compassionate and experienced  Case Manager  to join our team in providing high-quality, patient-centered careacross the continuum. In this role, you’ll coordinate care for clinically complex patients, ensuring effective treatment sequencing, resource management, and comprehensive discharge planning. You’ll play a key part in achieving the best possible clinical and cost outcomes for our patients.

Essential Functions and Responsibilities

• Coordinate and monitor patient care plans from admission through discharge
• Facilitate communication among interdisciplinary teams, patients, and families
• Evaluate treatment progress and ensure appropriate utilization of resources
• Develop and implement individualized discharge plans
• Serve as an advocate for patients’ rights and continuity of care
• Identify and resolve barriers to treatment goals
• Maintain compliance with all regulatory, safety, and performance standards
