The Position :

The Nurse Liaison plays a critical role in coordinating the transition of patients from hospital or healthcare facilities to home-based or alternate site infusion therapy. This position works closely with hospital discharge planners, case managers, physicians, and the internal clinical and operational teams to ensure safe, timely, and seamless discharges. The Nurse Liaison also provides direct bedside education and training to patients and caregivers on infusion therapy protocols, device use, and care coordination. Responsibilities Include :
