Job Description

Assess, plan, implement, and evaluate patient care under direction of RN or Physician.

Provide direct patient care consistent with policies and procedures.

Inform patients and families about treatments, medications, and equipment.

Document care provided in accordance with guidelines.

Collaborate with health care team to ensure continuity of care.

Participate in quality monitoring activities directed by RN or Physician.

Advocate for privacy and confidentiality of patient data.

Critical Information

Nights 1830p-0700a, 12-hour shifts, every other weekend.

On-call : None.

Work is based at a skilled nursing facility with no float sites.

Education / Licenses Needed

LPN License in the State of Michigan.

High School Diploma or equivalent required.

Completion of an approved nursing program required.

BLS certification from AHA or ARC within 90 days of hire.
