Job Description :

We are seeking a compassionate and skilled Occupational Therapist (OT) to provide rehabilitative care within a skilled nursing facility setting. The OT will evaluate, plan, and deliver therapeutic interventions to support residents in achieving improved functional independence and quality of life.

Responsibilities :

• Conduct comprehensive evaluations to determine patient needs and establish individualized treatment plans.
• Provide therapeutic interventions targeting strength, mobility, coordination, cognition, and daily living skills.
• Adapt treatment approaches based on patient progress, safety, and therapeutic goals.
• Collaborate closely with interdisciplinary team members to ensure cohesive patient care.
• Educate patients, families, and caregivers on exercises, adaptive techniques, and safety strategies.
• Document assessments, progress notes, and treatment outcomes in accordance with professional and facility standards.
• Recommend adaptive equipment and environmental modifications as needed.
• Maintain a safe and supportive therapy environment.
• Comply with all regulatory, ethical, and facility guidelines.
• Promote patient-centered care with empathy, professionalism, and clinical expertise.
