On-Call RN

We are looking for a weekend On-Call Registered Nurse (RN) or LPN to join our Hospice Team. Duties include telephone triage for calls coming in from 5pm to 8 : 30am. Visits to patient’s home, as needed. Communicate call notes to case managers. RN RESPONSIBILITIES :

The Registered Nurse (RN) or LPN provides skilled nursing care to clients in their homes under the supervision of the Director of Nursing, according to the physician’s orders, meeting the Hospice standards established by Medicare, within the scope of licensure requirements and following established standards and practices. Primary duties will include patient visits during weekend hours, as needed, as well as possible admissions.

RN REQUIREMENTS :

• Current RN or LPN license to practice in the state of operation.
• Minimum of one (1) year nursing experience. Experience in home health or hospice is preferred.
• Current CPR certification.
• Schedule flexibility (this position requires weekend availability) and travel required.
• Ability to communicate effectively verbally and in writing.
• Must have valid driver’s license, vehicle insurance, and reliable transportation.
