POSITION SUMMARY : The Community Health Nurse plays a crucial role in enhancing healthcare delivery by reviewing and implementing clinical workflows within physician practices to achieve PCMH level 3 recognition. You will identify a population of individuals with chronic diseases who require tailored healthcare interventions based on a comprehensive stratification process, using data from visits, claims, lab results, and admissions. Collaborating closely with clinical support teams, nursing staff, and physicians, you will help drive the system’s goals of improving health outcomes for vulnerable patients.
EDUCATION / CERTIFICATION : Graduate of an accredited nursing program; an Associate's degree in an applicable field is required, while a Bachelor's degree is preferred. A current license as a Licensed Practical Nurse is required.
EXPERIENCE : Minimum of five (5) years of nursing experience in a community health setting, specifically working with vulnerable populations.
COMPETENCIES : Strong analytical, organizational, and interpersonal skills are essential. You should communicate effectively in English, both verbally and in writing, and possess a solid working knowledge of Microsoft Office products. The ability to multitask and operate independently is essential.
