We are seeking a dedicated Nutrition Patient Care Coordinator to join our team and enhance patient care through effective nutrition management. In this role, you will play a crucial part in patient follow-up and evaluating adherence to physician orders.
Key Responsibilities :
- Provide education to patients tailored to their needs, ensuring adherence to company policies.
- Conduct thorough and professional patient assessments in line with established procedures.
- Stay current with enteral home care techniques and relevant therapy concepts.
- Represent our organization through professional appearance and quality patient care.
- Collect necessary information from referral sources to secure insurance coverage for enteral patients.
- Manage paperwork to facilitate payment for rendered services.
- Maintain organized and comprehensive patient records in compliance with accreditation standards.
- Deliver enteral formula, feeding pumps, IV poles, and related supplies to patients in their homes or workplaces.
- Ensure continuity of patient care across various settings to meet individual needs.
- Promote a safe and effective environment for both patients and organizational operations.
- Engage in ongoing education, including training and in-services, to continuously develop your skills.
- Read and interpret safety rules and operational instructions to ensure compliance and safety.
- Calculate figures and solve practical problems involving a variety of concrete variables.
- Interpret diverse instructions presented in various formats (written, oral, diagram, etc.).
Qualifications :
One year of experience in home health care, hospital, clinic, or nursing home preferred.Education or experience in Dietetics, Nutrition, Home Health, or Nursing is strongly preferred.A minimum of one year of experience as an LPN, DTR, or CNA is preferred.Excellent interpersonal skills to interact effectively with doctors, referral sources, and patients.Physical Demands :
