HealthCare Support is actively seeking multiple Licensed Social Worker Case Managers to fill multiple openings with a Managed Care Organization in Wayne and Macomb Counties .

Daily Responsibilities for LSW Field Care Manager Behavioral Health :

• Assesses and evaluates members' needs and requirements to achieve and / or maintain optimal wellness state by guiding members / families toward and facilitate interaction with resources appropriate for the care and wellbeing of members.
• Shift : M-F 8AM-5PM
• This is a HYBRID position with Heavy Field Work and Remote Work
