Key Responsibilities

Assessment and Planning

Case Managers conduct comprehensive assessments to understand the unique needs of their clients, including medical, social, emotional, and educational dimensions. This involves collaborating with clients, families, and other service providers to develop individualized care plans that address specific goals and effectively meet each client's needs.

Service Coordination

Case Managers ensure clients have access to the appropriate community resources, such as healthcare providers, therapists, educational programs, and social service agencies. Responsibilities include handling referrals, coordinating appointments, and ensuring timely delivery of services tailored to client needs.

Advocacy

A Case Manager champions the rights and needs of their clients within various systems. They work to eliminate barriers, advocate for essential resources, and ensure that clients have access to the services necessary to support their well-being.

Progress Monitoring and Evaluation

To ensure service effectiveness, Case Managers regularly monitor client progress, evaluate the success of care plans, and implement necessary adjustments. Accurate documentation of interactions and outcomes is a key part of this role, ensuring compliance with program regulations while maintaining continuity of care.

Areas of Focus

  • Childrens Mental Health

Work with families of children experiencing mental health challenges to coordinate therapeutic and supportive services that improve overall well-being.

  • Adult Mental Health
  • Assist adults with mental health conditions by helping them access benefits, employment opportunities, medical care, and other critical services.

  • Substance Use Disorder
  • Facilitate treatment and support for individuals dealing with substance use disorders. This includes assessing needs, creating service plans, and coordinating treatment programs.

  • Developmental Disabilities
  • Support individuals with intellectual and developmental disabilities by promoting independence and active participation in community life.

    Qualifications

  • Degree in social work, nursing, counseling, or a related field.
  • Knowledge of community resources and service delivery systems.
  • Strong interpersonal and communication skills.
  • Ability to work independently and as part of a team.
  • Excellent organizational and time management abilities.
  • High level of empathy and compassion to support clients effectively.
  • Work Hours and Compensation

  • Schedule : Monday through Friday, 8 AM to 5 PM.
