A well-established, growing primary care clinic in Fresno, CA, supported by a rapidly growing healthcare management organization, is seeking an MD (preferred) or NP / PA with strong experience in primary care for geriatric or senior populations . This clinic currently operates with two physicians and serves a primarily HMO patient base, including annual wellness visits and chronic disease management. The ideal candidate is patient-focused, efficient, and comfortable managing a high-volume outpatient panel of 20-30 patients per day in a structured, well-supported clinical environment.
Responsibilities
- Provide comprehensive primary care to an adult and geriatric patient population.
- Deliver in-clinic primary care, including evaluations, follow-ups, preventive care, and chronic disease management
- Collaborate closely with clinic physicians, medical assistants, case managers, and operational staff.
- Deliver high-quality, evidence-based care aligned with HMO standards, preventive guidelines, and organizational protocols.
- Maintain accurate and timely documentation within the EMR system.
- Coordinate care with specialists, hospitals, and care-management programs as needed.
- Participate in quality initiatives, patient-outcome programs, and performance metrics.
- Contribute to a positive and collaborative clinic culture focused on senior care excellence.

Requirements
Active, unrestricted California license as an MD / DO , NP , or PA ; Board Certified / Eligible (MD / DO) or nationally certified (NP / PA).2-4 years clinical experience in primary care , internal medicine, family medicine, or geriatric / senior care , including management of chronic conditions.Experience with Medicare Advantage , HCC coding, preventive care, and Annual Wellness Visits (preferred).Ability to manage a high-volume outpatient schedule of 20-30 patients per day while maintaining high-quality, patient-centered care.Proficiency with EMR systems and commitment to accurate, timely, and compliant documentation.Strong communication skills with the ability to educate patients and collaborate effectively with interdisciplinary clinical teams.Excellent clinical judgment, professionalism, and reliabilitySalary : MD : $220-280K ; NP / PA : $130-180K .
