• This is a full-time, Monday through Friday, Daytime position.
• This position requires a valid Pennsylvania Licensed Practical Nurse license, a valid Pennsylvania driver's license, and reliable internet access.

As a full-time Homeland Hospice LPN, you would be eligible for :

As Homeland Hospice's Licensed Practical Nurse (LPN) you would provide skilled nursing care to the hospice patient, and support to the family and caregiver, as directed by the supervising Assistant Director of Clinical Services, in compliance with Medicare Hospice Conditions of Participation, Pennsylvania licensure laws and agency policies and procedures. Provide timely documentation of patient services based on the patient's plan of care. Works closely with other members of the Interdisciplinary Team (IDT) to ensure all patient and family care needs are being met.

Requirements include :

Graduate of accredited practical nursing program.

LPN licensed in the Commonwealth of Pennsylvania.
