POSITION DESCRIPTION : Physical Therapist

  • Qualifications :
  • A graduate of a professional physical therapy education program accredited by a national accreditation agency approved by the Arizona Physical Therapy Board
  • Current licensure from the Arizona Board of Physical Therapy

Terms of Employment : Ten Month

  • Duties and Responsibilities :
  • Screen and evaluate appropriately referred students within the State and Federal requirements.
  • Attend and actively participate in all appropriate Child Study Team (C.S.T.), Section 504, Multidisciplinary Evaluation Team (M.E.T.), and Individualized Education Program (I.E.P.) meetings.
  • Develop physical therapy related IEP / 504 goals, objectives, and / or benchmarks, including any appropriate accommodations for qualified district students as assigned.
  • Complete all State and Federally required paperwork within required timelines.
  • Provide individual and group physical therapy services to students as described in the IEP and 504 Plans.
  • Prepare quarterly, or more frequent if needed, progress reports on each assigned student to include in the quarterly grade reporting process.
  • Complete all MEDICAD reports and requirements in a timely manner.
  • Participate in District training as appropriate.
  • Perform other related duties as may be assigned by the Director of Special Services.

  • The employee must occasionally lift and / or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
