Salary Range : $60,000-$72,000 annually

After six months of employment, staff may be eligible to receive a retention bonus of $3,000.

The Youth ACT Team serves children / youth with Serious Emotional Disturbance (SED) throughout Westchester County, who are returning home from inpatient settings or residential services, at risk of entering such settings, or have not adequately engaged or responded to treatment in more traditional community-based services. Youth ACT team delivers intensive, highly coordinated, individualized services and skilled therapeutic interventions through an integrated, multi-disciplinary team approach to better achieve success and maintain the child in the home, school and community.

Summary Description :

The Licensed Mental Health Professional - Youth ACT plays a pivotal role in providing mobile and community-based mental health services and support to youth in alignment with the Youth ACT program. This position focuses on addressing the unique mental health needs of young individuals, fostering resilience, and promoting overall mental well-being.

Key Responsibilities :

• Provide individual and group counseling services to youth participants of the Youth ACT program in the home and community.
• Collaborate with the Youth ACT Team, school personnel, parents, and external partners to assess and address the mental health needs of youth.
• Conduct comprehensive mental health assessments to determine treatment goals and intervention strategies.
• Develop and implement age-appropriate therapeutic interventions to support youth in achieving their mental health goals.
• Participate in case conferences and collaborate with multidisciplinary teams to create holistic care plans for youth participants.
• Facilitate psychoeducational workshops and support groups to enhance mental health awareness and coping skills.
• Monitor and evaluate the progress of youth participants, adjusting interventions as needed.
• Maintain accurate and confidential client records, ensuring compliance with regulatory bodies and legal / ethical standards.
• Provide 24 hour crisis intervention and support to youth experiencing mental health emergencies on a rotating basis.
• Stay informed about best practices and evidence-based interventions in youth mental health.

This is a Mandated Reporter position.
