Location : The Village of Meyerland, an Assisted Living with Memory Care senior community.
Essential Duties and Responsibilities
- Administer medications safely and accurately per physician orders and regulatory standards
- Oversee medication aides and conduct monthly medication reconciliation and audits
- Coordinate daily staff assignments to ensure consistent resident care
- Communicate effectively with physicians, families, and care teams regarding resident needs
- Provide direct care including ADLs : bathing, dressing, grooming, toileting, mobility, and incontinence support
- Respond to medical and behavioral emergencies with sound clinical judgment
- Complete clinical documentation including assessments, evaluations, and service plans
- Document incidents and communicate promptly with leadership
- Monitor care team documentation for accuracy and completeness
- Support orientation and ongoing training of care staff
- Assist with scheduling in collaboration with leadership
- Attend required in-services and continuing education
- Maintain professional appearance and hygiene-
- Promote respectful and effective communication across teams and families
- Perform other duties assigned to support resident care and operational excellence
Minimum Requirements :
Current and valid LVN / LPN license in the state of employmentBasic Life Support (BLS) / CPR certificationMinimum 6 months to 1 year experience in Assisted Living or Long-Term CareProficient in Electronic Health Records (EHR) and basic computer skillsStrong verbal and written communication skillsDemonstrated critical thinking and problem-solving abilitiesKnowledge of HIPAA regulations and compliance standardsAbility to respond with patience and understanding to dementia-related behaviorsPhysical ability to perform essential job functions with or without reasonable accommodationEOE / M / F / D / V
