Director of Social Services – Skilled Nursing

We’re hiring a full-time Director of Social Services to oversee our Social Work Department for long-term and acute care patients. Help residents thrive, guide families, and lead a team dedicated to exceptional care.

What You’ll Do :

Lead social services for residents and families.

Develop social histories, assessments, and care plans.

Participate in interdisciplinary care meetings.

Support discharge planning and placement.

Ensure compliance with state and federal regulations.

What We’re Looking For :

Bachelor’s in Social Work or related field (Master’s preferred).

Strong interpersonal & leadership skills.

Experience in long-term care or healthcare social services.

Knowledge of state and federal social services guidelines.
