Registered Nurse Case Manager

A company is looking for a Full Time Registered Nurse, Adult Complex Case Manager (Very High Risk / Transition of Care) - Remote. Key Responsibilities Conduct thorough assessments of members' health concerns and prioritize medical needs Develop and communicate comprehensive personalized care plans in collaboration with members and their care teams Guide members to appropriate facilities and resources based on their medical needs Required Qualifications Bachelor's degree in nursing (BSN) or equivalent Current / active Certified Case Manager License (CCM) or commitment to obtain certification within one year of hire Minimum of 5 years as an active Registered Nurse with a current unrestricted license At least 2 years of direct care or case management experience in relevant specialty fields Minimum of 1 year telephonic case management experience
