Harlem Center for Nursing and Rehabilitationis looking for an RN AssessmentCoordinator to join the team at our Skilled Nursing Facility in Brooklyn, NY!

RN Assessment Coordinator Responsibilities

• Conduct and coordinatethe completion of MDS assessments
• Maintain and updateall care plans and assessments
• Examine and reviewresidents' medical history
• Inform other nursingstaff personnel of any persistent issues
• Participate in theadmission process while assessing prospective residents' nursing needs andappropriate placement
• Coordinate casemanagement process
• Ensure that thefacility adheres to all regulatory requirements and standards of practice inrelation to the MDS care planning process
• Cooperate with inter-departmental personnel to ensure services can be adequately maintainedto meet the needs of the residents
