Part-Time RN Case Manager

Position Overview :

Join our team as a Part-Time RN Case Manager! In this role, you will play a crucial part in coordinating patient care and ensuring optimal resource utilization while supporting patient satisfaction and appropriate length of stay (LOS).

Key Responsibilities :

• Assess patient physical, psychosocial, and economic needs for effective transition of care planning.
• Facilitate development and communication of the continuum of care transition plan in collaboration with the care team.
• Document and validate specific data to monitor outcomes and prevent readmissions.
• Ensure compliance with accreditation and regulatory requirements while integrating performance improvement principles into your responsibilities.
• Obtain necessary pre-certifications and manage authorizations for hospitalizations and post-acute services.
• Address clinical denials and collaborate with various teams to formulate improvement plans.
• Maintain up-to-date knowledge of relevant care management practices and serve as an educational resource for staff.
• Communicate effectively with outpatient providers to enhance the continuum of care.
• Commit to professional growth and required continued education.
• Perform additional duties as needed.
