What You Will Do as a Hospice Network Liaison

As a Hospice Network Liaison, you will match service locations capabilities to patient needs and facilitate patient flow from acute care providers into Stonerise to meet / exceed census and goals. Some of your primary responsibilities will include :

  • Communicating with the Interdisciplinary team daily to ascertain information and knowledge about open beds, scheduled discharges, scheduled admissions, hospitalized patients, clinical capabilities, and center events
  • Managing the electronic referral system for assigned centers and develop professional relationships with case managers and discharge planners, hospital employees and management, patients, and families
  • Completing clinical reviews via Allscripts and / or onsite evaluation as needed or requested, and coordinating a seamless transition to assigned centers
  • Remaining visible as assigned hospital and being available as a resource to promote Stonerise in a positive manner
  • Abiding to all HIPAA laws and observing confidentiality policy
  • Presenting the benefits of long and short-term skilled nursing at Stonerise to patients, families, hospital employees, etc.

    Full Time

    Education, Licensure & Certification Requirements

  • Two (2) years of experience in sales and marketing
  • Current and valid drivers license with available transportation and ability to drive
