Landmark Hospital of Athens has an exciting opportunity for a discharge planner to join our dedicated and professional team. Landmark Hospitals are committed to providing the highest level of customer service and quality of care to those we serve.
Summary : The role provides discharge planning and coordination for a group of patients in support of the social work and case management departments. Works directly with patients, families, and referral / community resources to address the various needs or barriers surrounding patient discharge planning (financial, clinical, psychosocial, logistical issues, etc.) This position also aids with communications with external agencies to provide referral information for a smooth transition from the LTACH hospital to the post-hospital setting. Works closely with admission coordinators and case management / social work team. Performs other duties as assigned.
Performs a variety of tasks related to the coordination of care. A certain degree of creativity and latitude is required. Ensures that the patients' unique needs are met in the most appropriate, timely, and cost-effective manner possible. The ideal candidate must be highly detail and goal-oriented, with superior analytic, critical-thinking and time-management skills. They must have a positive and empathetic personality with the ability to work effectively with others as part of a multidisciplinary team in complex care environment. It is also vital that they have excellent communication and interpersonal skills and consistently display a professional attitude.
