job_description.job_card.job_descriptionProficiency in HomeCare HomeBase (HCHB) EMR, used within the last 6 months, or completion of a retraining course during compliance Proficiency in OASIS Provide, coordinate, and direct the provision of home nursing care according to physicians' orders and agency policies through the competent application of the nursing process Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses Conduct home visits and attend periodic office meetings Manage an average of 5-8 patients per day Achieve weekly productivity expectations of 30 points, with point values assigned for each visit and activity Types of cases include Start of Care, Wound Care, Follow-up Visits, etc. Willingness to travel and cover a territory radius of up to 75 miles from the home clinic Flexibility to work in a standard 5x8 schedule primarily from Monday to Friday, with the possibility of a weekend rotation 1-2 times per month On-call duties required 1-2 times per month Responsible for completing all documentation within 24 hours of patient appointments Must have reliable transportation, a current driver's license, and agency-required liability insurance Mileage reimbursed at IRS standard rate; travel compensated from patient home to patient home
