Position Overview

As an RN Case Manager, you will play a critical role in delivering compassionate, patient-centered end-of-life care. Your responsibilities will include overseeing the clinical care of a designated group of hospice patients, ensuring their physical, emotional, and spiritual needs are met in alignment with their goals and wishes.

Key Responsibilities

  • Coordinate hospice care in collaboration with team members and patients' families to ensure their needs are met.
  • Continuously evaluate the effectiveness of hospice services for patients and families.
  • Conduct evaluations for hospice care and informational visits as needed.
  • Utilize the nursing process to deliver professional nursing care.
  • Ensure coordination of patient services with other members of the Interdisciplinary Group (IDG) as well as contracted services.
  • Submit comprehensive, accurate, and timely clinical documentation.
  • Participate in IDG meetings and case conferences.
  • Educate patients and families about the dying process, care planning, and provide guidance on healthcare decisions.
  • Perform on-call duties according to policy and organizational need.
  • Develop and implement the initial plan of care, collaborating with IDG members to maintain a comprehensive plan of care.
  • Document the services rendered and changes in patient conditions, adjusting the nursing care plan as necessary.
  • Build and maintain strong relationships with patients, families, physicians, and assisted living communities.
  • Qualifications

  • Must be a licensed Registered Nurse in Arizona.
  • Minimum one year of RN experience, with hospice experience preferred.
  • Strong assessment and problem-solving skills.
  • Excellent organization and time management capabilities with a solid understanding of case management processes.
  • Licensed driver with an insured and well-maintained automobile.
  • Effective verbal and written communication skills.
  • No exceptions to the RN license, capable of administrating pharmaceuticals, performing medication reconciliation, and executing nursing procedures as required.
  • Proficient with electronic medical records (EMR) and related technologies.
