Job Title : Licensed Practical Nurse - Hazel Cypen Tower

Job Summary

Provides direct patient care, meeting both psychological and physical needs of the patients in accordance with their care plan, regulatory agency, and departmental guidelines. Maintains standards of nursing care, under the supervision of registered nursing personnel, in accordance with the philosophy, goals and objectives of the Nursing Department and regulatory agencies. Provides work direction to Certified Nursing Assistants.

Essential Job Functions

• Performs nursing care for assigned patients / residents per established department guidelines, policies, and procedures.
• Handles admissions, discharges, and transfers of assigned residents / patients.
• Shares responsibility for the planning, implementation, and evaluation of nursing care plans and nursing care.
• Performs documentation according to established standards and procedures.
• Prepares, administers, and concisely, accurately, and completely records medications in accordance with the policy regarding the administration of medications.
• Receives, transcribes, and carries out physicians’ orders.
• Oversees and assists with meal service.
• Oversees, assists, and guides the work of CNAs in the daily activities.
• Provides coaching to CNA’s and participates in the annual performance appraisal process.
• Assists in the orientation of new personnel when requested.
