This Social Worker serves as a direct practice clinical social worker in an acute care setting serving populations that include veterans receiving care on surgical and medical wards as well as surgical intensive care, medical intensive care and step-down areas and is located at the West Haven VA Medical Center in West Haven CT. Responsibilities duties of this position include but are not limited to : independently performing case finding functions and outreach, identifying and screening patients for psychosocial needs. Identifies high-risk patients and provides case management services. Provides consultation services to other staff regarding the psychosocial needs of veterans and the impact of psychosocial problems on health care and adherence to treatment plan. Establish and maintain effective therapeutic relationships with veterans and their families. Ability to identify, utilize and refer to VA services that promote health and wellbeing such as Substance Abuse Day Treatment, MOVE program, Home telehealth, mental hygiene, Fee Home Health Aide program, Contract Nursing Program. Employ appropriate treatment modalities needed to engage effectively with clients. Participate in the Social Work 24-hour emergency on call coverage program as needed; providing telephone consultation, or onsite social work intervention if the clinician evaluates that to be required. Work Schedule : Full Time 40 Hours per week, Sunday 730am-4pm, Monday-Thursday. 8am-430pm Compressed / Flexible : Not Authorized Retirement : Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA Insurance : Federal health / vision / dental / term life / long-term care (many federal insurance programs can be carried into retirement) Telework : Available.
