Position Summary

The Program Associate will be reporting to the Program Manager. This person will ensure smooth coordination of supportive services for families, including Transitional Age Youth (TAY), in UBHs DMH-funded interim housing program. This role supports referrals, intakes, and documentation, while helping families transition from crisis housing to permanent stability.

Key Responsibilities

  • Referral & Intake : Manage incoming referrals, schedule intakes, and ensure timely documentation.
  • Documentation & Data : Maintain accurate case files, HMIS entries, rosters, and reporting.
  • Service Coordination : Partner with case managers to track family progress and flag service gaps.
  • Partner Communication : Liaise with DMH, CES agencies, and community partners to meet program requirements.
  • Program Support : Assist with internal coordination (room readiness, supplies, meal tracking) and attend team meetings.
  • Family Engagement : Foster a trauma-informed, family-centered environment that promotes housing stability.

Qualifications

  • 12 years experience in social services, housing, youth, or family support (TAY experience preferred).
  • Knowledge of DMH / CES programs strongly preferred.
  • Strong communication, organizational, and administrative skills.
  • Proficiency with online case management systems (HMIS a plus).
  • High school diploma required; AA / BA in social work, human services, or related field preferred.
  • Bilingual (Spanish / English) and familiarity with LA County housing systems is a plus.

  • Valid CA Drivers License, proof of insurance, and insurability under UBH policy.
  • Reliable transportation.
  • Current TB test and background clearance.
  • CPR / First Aid certification.
