Position Overview :

Join our compassionate and dedicated team as an On-Call Hospice Registered Nurse (RN)! In this vital role, you will provide exceptional, patient-centered care to individuals receiving hospice services. Your expertise will ensure that patients remain comfortable, safe, and well-cared-for during the most critical moments of their care. Key Responsibilities :

Quickly respond to emergency calls from hospice patients during on-call shifts.
Deliver direct nursing care and perform clinical assessments, focusing on symptom management, pain relief, and end-of-life care.
Collaborate with a dedicated interdisciplinary care team to develop and execute personalized care plans.
Educate and support patients and families regarding comfort measures, medications, and end-of-life care procedures.
Maintain timely and accurate documentation of all patient interactions and care provided.
Follow all hospice care policies, regulatory standards, and industry guidelines.

Qualifications :

Current and active Registered Nurse (RN) license in Utah.

Preferred prior experience in hospice or palliative care.

Strong clinical skills, particularly in symptom management including pain, nausea, and anxiety.

Outstanding communication skills with a focus on providing compassionate, empathetic care to patients and families.

Ability to work independently while managing multiple responsibilities in a fast-paced, on-call environment.

Reliable transportation and availability for on-call shifts, including nights and weekends.
