Nurse Practitioner Partner

A company is looking for a Nurse Practitioner Partner (Multiple States). Key Responsibilities Conduct 5-10 hours per week of telehealth visits, including consultations and medication prescriptions Collaborate with Community Health Workers (CHWs) to assess patient needs and eligibility for services Provide clinical oversight to CHWs and maintain accurate patient records Required Qualifications and Experience Board certification as a Nurse Practitioner Minimum of 2 years of consecutive experience as a Nurse Practitioner Commitment to health equity and addressing social determinants of health Ability to operate independently with strong diagnostic skills Experience in telemedicine and proficiency with various digital tools
