Physical Therapist (PT) - Part Time and Per Diem Opportunities

POSITION SUMMARY :

We are a premier home health provider in our community, enhancing care experience for the patients, families, and the referral sources we serve. The Home Health Physical Therapist is responsible for delivering personalized rehabilitation services to patients in their homes, focusing on improving long-term health and wellness. This role involves conducting comprehensive assessments, developing individualized treatment plans tailored to each patients goals.

RESPONSIBILITIES :

• Conduct an initial evaluation / assessment and report findings to the physician / provider and Clinical Manager and develop the plan of care promptly for patients
• Perform physical therapy interventions, procedures, measurements, evaluations and diagnostic tests on patients in the home
• Document initial evaluation and treatment plan and re-evaluations of patient's progress every thirty (30) days and updates plan of care as necessary
• Complete comprehensive assessments and OASIS data, as indicated on a timely basis
• Assist with orientation, supervision and consultation services to Physical Therapy Assistants
• Observe, record and report to Health Care Provider the patient's treatment, response and progress and promptly report significant changes and obtain orders to revise plan of care
• Communicate with physician / provider and Health Care Provider and other members of the health care team for the delivery and coordination of quality physical therapy services
• Provides education and support to patients and their families
• Other duties as assigned
