Padua Case Manager

What will you be doing in your role?
Identify opportunities for client growth and develop a plan to assist them in reducing barriers toConduct comprehensive assessments that incorporate a narrative approach in the development of strengths-based, holistic service plan through financial coaching and emotional resiliency to identify client BBF / goal / why.Monitor and guide client progress utilizing clinically informed interventions to help maintain client engagement and progress in achieving overall well- being and financial self-sufficiencyEnsure professional service delivery by working in collaboration with case work partner to provide timely case management, documentation, and communication withUtilize coaching approach to empower client while encouraging engagement and motivation to identify multiple paths towardsMaintain current knowledge of trends and issues regarding social work theory and bestUtilize a problem-solving framework to promote steady clientThrive on finding the approach that works best for each person you serve despite the intensity of the barriersEngage clients in service planning process so that they agree with and understand the goals, target dates, and actions needed toBuild and maintain a strong relationship with case worker partners to maintain a caseload that is engaged and successful in meeting client goals and agency's benchmarks.Maintain a flexible schedule to occasionally meet with clients after normal business.Connect clients to wellbeing / counselingHold yourself and team accountable for meeting expectations and are proactive about addressingContinually explore training opportunities to increase your skill set.Support the data and evaluative needs of Padua by ensuring proper, accurate and timelyCultivate positive relationships and collaborate with internal and external partners.Are you the next Padua Case Manager?
Bachelor's degree in social work or closely related field (master's degree and / or clinical experience preferred)Case management experienceExcellent communication and interpersonal skillsMust have reliable transportation, a current Texas driver's license and be willing to transport clients in your car if needed.Here's the Good Stuff...
Full-Time hourly position.
