About The Position

Job Title : Care Coordinator - Case Management Profession : RN (Registered Nurse) Specialty : Utilization Review Duration : 13 weeks Shift : Days Hours per Shift : 8 hours Experience : Minimum 3 years in acute hospital setting License : Required Certifications : BLS Must-Have : - CommonSpirit / Dignity / CHI experience - Excellent customer service skills - Strong interpersonal and written communication skills - Ability to multi-task and work independently - Ability to work efficiently in a fast-paced environment with changing priorities - Prior care coordination experience in an acute hospital setting Description : Must have experience dealing with all payers, uninsured, and homeless patients. Responsibilities include completing initial assessments and discharge planning. This includes nursing homes, home health, durable medical equipment (DME), and prior authorizations. Caseloads can range from 15-28 patients on average per day. Experience with electronic medical records (EMR) is required. Position primarily focuses on discharge planning. Experience in inpatient care is necessary. No call requirement for this position. Dress code is business casual with a lab coat.
