Responsibilities :

  • Administer nursing care to ill, injured, or disabled patients
  • Deliver care consistent with patient treatment plans
  • Monitor and report changes in patient symptoms or behavior
  • Communicate with physicians or specialists regarding patient care
  • Educate patients about health maintenance and disease prevention
  • Facilitate referrals to other healthcare professionals and facilities
  • Maintain accurate patient medical records
  • Provide advice and emotional support to patients and their families

    • Qualifications :

  • Active Georgia RN or LPN license
  • At least one year of home health care experience
  • BLS Provider CPR Certified
  • Experience in Chronic Disease Management (preferred)
  • Self-starter, highly motivated, and dedicated
  • Experience with feeding tubes, tracheotomies, or ventilators (a plus)
  • Strong problem-solving and critical thinking skills
  • Ability to thrive in a fast-paced environment
