We are seeking a highly skilled and compassionate Lead Registered Nurse (RN) in the Oak Ridge, TN area to join our team for assignments across the state of TN. This role allows you to stay close to home while gaining diverse experience, making a meaningful impact on patients' lives and contributing to different communities within the state. The Lead Registered Nurse (RN) will support patient care and oversee nursing operations in a private-duty home setting. This position combines hands-on care with leadership responsibilities, ensuring seamless communication between the patient, caregivers, and the Patient Coordinator.

This role is a Full-Time benefit eligible opportunity, with care being provided at the patient's residence. Patients range from high acuity to those who only need basic medical management.
Once our patients are accepted into the program, there is no expiration for this benefit, which allows our team of caregivers to build lasting relationships with their patients for years to come.
You'll get to advocate for the patients and work one-on-one to provide them with the care that they need.
This role will collaborate with CNA's, HHA's, Case Managers, and the patient's physicians.
Offers are contingent upon passing a background check.
Shift : Full-Time, Day Shifts (Minimum of three 12-hour shifts per week. Monday through Friday).

Responsibilities
- Execute physician prescribed plans of care.
- Provide direct patient care while serving as the Lead Nurse.
- Work minimum of three 12-hour shifts per week.
- Ensure all shifts are filled to maintain 24-hour patient coverage.
- Secure coverage for open shifts or call-outs; if coverage cannot be found, the Lead Nurse will work the shift.
- Maintain and update the patient's schedule in Google calendar.
- Submit monthly schedules by Week 3 of each Month.
- Ensure complete and timely charting by each team nurse after every shift.
- Coordinate patient needs and concerns with the Patient Coordinator.
- Complete compliant documentation of the care provided in the system of record.
- Manage and administer prescribed medication, treatment, and therapies.
- Conduct patient assessments, coordination of care
- Perform various activities associated with daily living.
- Monitor vitals and GI intake and output.
- Assess skin integrity and administer wound care.
- Monitor for changes in the patient's condition such as weight loss / gain, self-care abilities, and indicators of disease progression.
- Communicate with family members, physicians, case managers, and other prescribed individuals regarding the health of the patients.
- Provide education and training to the patient, their family, and / or the Home Health Aide.
- Work with patients to be done one-on-one in the patients home to provide for a comfortable environment.
- Assist with office tasks including but not limited to : Intake assessments, HHA skills assessments, clinical chart audits, supervisory visits, short term temporary nursing assessments, schedule nurse staffing, completing a CPR competent checklist / assessment for caregivers, educate and train HHA, CNA and LPN's, participate in the office compliance program.
- Perform other personal care services as necessary to meet the patient's needs.
Minimum Qualifications
Education, licensing, and certification : graduate of an accredited college or university, Associate or Baccalaureate School of NursingCurrent State or Compact License as a Registered NurseCurrent CPR certificationTB test and physical examinationMaintain required insurance.Good interpersonal skillsAbility to stand or walk for prolonged periods, with the ability to lift up to 50 lbs and move patients.Ability to operate under stressful conditions and make quick decisions.Must have dependable transportation and be willing to commute to the patient's home.Ability to provide basic medical management up to medically fragile end of life care.Travel required - Travel throughout the state of TN as needed to fill short term patient assignments.Preferred Qualifications
