Our client, an established and growth-oriented provider organization, needs an experienced physician to direct post-discharge clinical care for a select patient population.

We are seeking a physician who is innovative and passionate about identifying ways to improve patient care. As a key member of the Priority Care Team, you will manage patients as they transition from inpatient care to a community or home-based setting.

Collaborating with leadership in Population Health and Case Management, you will provide high-quality, transitional care for more complex patients.

RESPONSIBILITIES :

• Directs patient care in outpatient clinic and / or home settings.
• Provides high quality care to patients as they transition from hospital to home.
• As an integral member of the Care Coordination Team, works closely with PCPs, Advanced Practice Clinicians, Social Workers, Case Managers, and Pharmacists
• Facilitates interdisciplinary meetings to ensure optimal coordination of care.
