Behavioral Health Care Coordinator

A company is looking for a Behavioral Health Care Coordinator to support the delivery of culturally responsive behavioral health services.

Key Responsibilities

Manage scheduling workflows for behavioral health appointments and coordinate referrals

Complete intake screenings and ensure timely follow-up on referrals

Support continuity of care and connect patients to community resources

Required Qualifications

2+ years of experience in care coordination or behavioral health administrative support

Experience working with culturally diverse or underserved communities preferred

Familiarity with behavioral health concepts and confidentiality regulations (HIPAA)

Bilingual (English / Spanish) required for communication in clinical and administrative contexts

Experience with electronic health records and comfort using spreadsheets or dashboards
