Hospice RN Case Manager

Moments Hospice is a locally owned and operated hospice. We are looking for an outstanding nurse case manager (RN) to join our team. Duties : What you will do :

• Provide skilled nursing care
• Provide case management services
• Conduct initial assessments of new patients
• Travel to patients wherever they reside : Home, nursing facility etc.
• Participate in an on-call schedule
