Provide palliative nursing care and support to pediatric patients and families

WHAT YOU WILL DO :

• Assess physical, psychosocial and spiritual needs of the patient and family.
• Provide intermittent skilled nursing services to the patient under the patient's physicians' directions.
• Monitor and evaluate pain and symptoms and provide information and assistance to the patient and family to manage those problems.
• Serves as liaison to the patient's physician(s) when needed; provide information to the physician based on knowledge relating to the patient's needs.
• Provide emotional support to the patient and family.
• Refer and follow up patients and / or family members to other services when appropriate.
• Provide instruction (health teaching) to the patient and family as related to home care.
• Maintain and promptly complete all necessary plan and reports related to home care.
• Participate in care conferences and bereavement meetings.
• Participate in public relations activities when necessary.
• Participate in audit review, quality assurance activities and other meetings as assigned.
• Plan and supervise activities of HHA as related to patient care.
