Description : NURSE PRACTITIONER

SUMMARY : As part of a collaborative agreement with a physician as mandated by Ohio law and under the direction of the AxessPointe Community Health Center, Inc. (API) Medical Director, the NP / PA will provide quality primary medical and behavioral health services to adults, children and families in a holistic manner within the northeast Ohio area.

SUPERVISOR : Medical Director or Designee

DUTIES AND RESPONSIBILITIES :

• Examines, diagnoses, and treats illnesses and injuries of patients throughout the lifespan.
• Provides well-child, well-woman and well-man health maintenance exams to new and established patients.
• Orders appropriate screening and diagnostic lab and radiographic exams.
• Interprets results of physical exams and screening / diagnostic tests to implement appropriate care plans for the patient.
• Prescribes medicines, therapeutic agents and treatment modalities as required by patient needs and in accordance with licensed prescriptive authority.
• Supports health promotion, health education, health empowerment and disease prevention interventions for all patients.
• Provides age-appropriate care to all patients.
• Promotes patient and family centered care through a collaborative team approach.
• Participates in established call group as necessary.
• Refers patients to specialty practices as appropriate and needed. Provides medical care to patient as part of mutual patient care with specialist.
• Confers with collaborating physician about patient care as necessary.
• Documents and maintains patient records of services provided according to program standards.
• May instruct medical students or nursing students.
• Operates within a scope of practice as agreed to by the FNP / PA, the collaborating physician, the medical director and in compliance with Ohio state law.
• Supervision and Administrative experience helpful

This position requires compliance with API's written standards, including its Compliance Program and Standards of Conduct and policies and procedures. Such compliance will be an element considered as part of the regular performance evaluation.

Failure to comply with API's Written Standards, which may include the failure to report any conduct or event that potentially violates legal or compliance requirements or API's Written Standards or, for managers and supervisors, fails to detect non-compliant conduct where reasonable efforts would have resulted in detection, will be met by the enforcement of disciplinary action, up to and including possible termination, in accordance with API's Compliance Program Policy and Procedure : Addressing Instances of Non-Compliance through Appropriate Disciplinary Actions.

While performing the duties of this job, the employee is frequently required to use close vision (clear vision at 20 inches or less). The employee will occasionally be working near moving mechanical parts, be exposed to outdoor weather conditions and exposed to a risk of electrical shock while running the copier, printer, other standard equipment and answering the telephone. The noise level in the work environment is low to moderate. Stress level can be high at times.
