Boulder Community Health is seeking a Clinic RN for the Behavioral Health Services Counseling Center who will be responsible for management of outpatient psychiatry program and new and existing patient triage under the supervision of the psychiatrists and psychiatric advanced practice providers. Manage the provision of individual patient mental health services and coordinate / collaborate with the clinical staff to maintain or revise the patient care plan under the direction of the physician. Communicate regularly with the patient, family, physicians, pharmacies, and payers as appropriate.

The Outpatient Counseling Center at Boulder Community Health (BCH) provides mental health and substance-use disorder care for patients, with the help of a multidisciplinary care team including psychotherapists and social workers working proactively to coordinate and manage care for each individual patient.

Qualifications

  • Colorado RN license required in good standing.
  • BLS certification required by the end of the 90-day consitional period
  • Previous nursing and medical experience required.
  • Inpatient or outpatient psychiatric nursing experience is preferred.

    • Responsibilities

  • Electronic Medical Record auditing, including, but not limited to, order management, tasking and messaging, appointment recall, and inbound fax management to ensure efficiency and patient safety.
  • Telephone triage calls for triage / referral in consultation with the appropriate clinic provider(s).
  • Retrieves, reviews, and critically assesses phone or other messages; then responds to them in a timely manner. Documents all incoming and outgoing communication.

    • Serves as a resource to clinic staff regarding medical information, patient assessment, and nursing management of illness.

  • Care coordination, including case management, referrals, and managing patient safety issues.
  • Overseeing patient safety in the clinic when placed on a 72-hour mental health hold (M1 hold) and assisting in transfer to the Emergency Department or inpatient unit.
  • Works with providers and patients to assess the urgency of patient problems. Coordinate scheduled appointments based on need in consultation with the appropriate clinic provider(s).
  • Assesses and provides individual status and care / instructions needed by the patient as indicated.
  • Consistent and on-time attendance is an essential function of the position.
  • Monitor labs and notify provider when outside expected limits.
  • Performs select nursing duties for patients ages starting at 18 years to geriatric, as well as select administrative duties.
  • Evaluate possible detox direct admissions in collaboration with an on-call provider.
  • Provides patient psychoeducation and instruction as necessary.
