Boulder Community Health is seeking a Clinic RN for the Behavioral Health Services Counseling Center who will be responsible for management of outpatient psychiatry program and new and existing patient triage under the supervision of the psychiatrists and psychiatric advanced practice providers. Manage the provision of individual patient mental health services and coordinate / collaborate with the clinical staff to maintain or revise the patient care plan under the direction of the physician. Communicate regularly with the patient, family, physicians, pharmacies, and payers as appropriate.
The Outpatient Counseling Center at Boulder Community Health (BCH) provides mental health and substance-use disorder care for patients, with the help of a multidisciplinary care team including psychotherapists and social workers working proactively to coordinate and manage care for each individual patient.
