Are you passionate about making a lasting impact in the lives of individuals and families transitioning out of homelessness? A leading nonprofit in Indianapolis is seeking a dedicated Housing Stabilization Case Manager to join their Streets to Home Initiative. This full-time, salaried position offers the opportunity to provide intensive, housing-focused support to those moving from street homelessness into permanent housing.
Key Responsibilities :
Manage a caseload of approximately 15 households, supporting clients from move-in through at least 12 months to ensure long-term housing stability.
Conduct regular home-based visits and develop individualized housing stability plans tailored to each household’s unique needs.
Connect clients with vital resources, including income supports, healthcare, transportation, and social services.
Collaborate with landlords, housing navigators, and community partners to address tenancy risks and promote successful outcomes.
Maintain accurate records in the Homeless Management Information System (HMIS) and adhere to Continuum of Care standards.
Participate in daily and weekly case management meetings and contribute to ongoing program improvement.
Provide transportation to clients as needed and engage in community-based work up to 75% of the time.
Qualifications :
3–5 years of experience in case management or direct service work with individuals experiencing homelessness or housing instability.
Knowledge of housing-first and eviction prevention strategies.
Experience supporting clients with wraparound needs such as healthcare, behavioral health, disability benefits, food assistance, and employment.
Familiarity with service coordination and electronic case management databases (HMIS or similar).
Strong organizational, communication, and interpersonal skills; ability to manage a caseload of 15–20 households.
Commitment to trauma-informed, client-centered, and equity-based practices.
Proficiency in Microsoft Office and Google applications; ability to master electronic databases.
Valid Indiana driver’s license and proof of auto insurance required.
Preferred Skills :
Experience collaborating with landlords, housing navigators, and multidisciplinary service teams.
Bachelor’s degree preferred.
Knowledge of Indianapolis community resources related to housing, healthcare, employment, and social services.
Background in trauma-informed care, harm reduction, and motivational interviewing.
Bilingual skills (English / Spanish or other languages commonly spoken in the community) are a plus.
Working Conditions :
Community-based role with some office / remote work.
Expectation of working remotely 25% and in the community 75%.
Travel required up to 75%.
