Position Summary :
Eventus WholeHealth delivers an integrated model of care to adults who reside in skilled nursing and assisted living facilities through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants and in-house Support Staff.
Essential Duties and Responsibilities :
Deliver high quality palliative care to patients with medical necessity for palliative service via in person and telehealth options. Assist with design of operational processes associated with development of palliative care program.
Work with facilities and Eventus employees to ensure the growth of palliative care program. Train and educate internal employees on palliative care principles and processes. Recognize gaps in processes and work with interdepartmental team members to streamline / organize palliative operations.
