Types of Cases : Alzheimer s disease, dementia, cognitive impairment, memory evaluations only

Credentialing : 30-60 days; temporary privileges may apply

Minimum Requirements for Consideration

Board Certified / Board Eligible : BC Only Neurology with Memory / Cognitive / Behavioral subspecialty

Licensure : Active CA medical license & DEA
