Annual Compensation : $60,000 - $70,000
Position Overview : Serves as Clinical Case Manager to provide comprehensive psychological and behavioral case management services within a continuum of services designed to improve or maintain a patient’s ability to function in the community. This position serves as a member of a multidisciplinary team to provide assessment, planning, implementation, coordination, evaluation, and monitoring of patients for behavioral health options and services.
Essential Responsibilities :
Supports continuity of care for behavioral health clients. Serves as an advocate for clients, families, and the behavioral health care team to facilitate positive outcomes.Conducts a comprehensive assessment of client’s health needs to develop a plan of care.Consults with Nurse Case Manager complex cases and trends.Facilitates communication and coordination between members of the healthcare team.Educates both the client / family and members of the behavioral healthcare delivery team about case management, healthcare and treatment options, community resources, insurance benefits, and psychosocial concerns so that informed decisions can be made.Serves on multidisciplinary teams; compliance, risk management, TMS workgroups, etc. Identifies potential case management clients through health planning assessments, review of databases, and verbal or written communication from behavioral health team members.Negotiates with internal / external providers / vendors for services and products in support of client’s needs.Supervisory or Managerial Responsibility :

Local travel to Family Care Center clinic locations in San Antonio and Austin.Minimum Qualifications :
Bachelor’s degree in psychology, social work or other related human services degree required.Two years of experience in mental health or human services required.Work Environment and Conditions :
Frequently required to stand and walk; continually required to sit.Frequently required to utilize hand and finder dexterity.Occasionally required to climb, balance, bend, stoop, kneel or crawl.Continually required to talk or hear.While performing the duties of this job, the noise level in the work environment is usually quiet.Occasionally lifts or moves more than ten pounds.Specific vision abilities required by this job include close vision; distance vision; color vision; peripheral vision; depth perception and ability to adjust or focus.Location : San Antonio and Austin Regions
