Salary : Starting at $80,000

Description :

The  Wellness Nurse to monitor resident health, implement wellness plans, and coordinate with care teams to promote a safe and healthy environment.

Responsibilities :

Assess and monitor resident health, including vital signs and chronic conditions

Administer medications and treatments within scope of practice

Collaborate with physicians, nurses, and care teams on wellness plans

Educate residents and staff on health, nutrition, and preventive care

Maintain accurate health records and document interventions

Support resident engagement in wellness programs

Qualifications :

Active LPN or RN license in Illinois

Experience in assisted living, long-term care, or senior care (Required)

Knowledge of geriatric care and wellness principles

Strong communication, organizational, and teamwork skills
